Thank you for your tremendous commitment and dedication to providing care to patients and the community, particularly in combating COVID-19. As a nonprofit social service organization, CISC is working to support the health care system by addressing essential needs and informing the communities we serve about ways to safeguard their health and well being through our bilingual staff.
CISC would like to inform the Senior Care Coalition membership about our ability to support COVID-19 patients who are being discharged from hospital or other care facility and back to their communities. With the experience patients endured due to infection, we know that community-based supports are critical in the recovery process including their physical and mental well-being. Through a coordinated approach, our team is standing by to support the needs of the patients, such as follow-up treatment, counseling, basic needs, employment, housing, and long-term-care, through short-term care coordination, family caregiver support, or long-term care case management.
CISC is able to assist patients who speak Chinese – Cantonese, Mandarin and Toishanese who are being discharged. Referrals can be made to:
